Ø Provides a residence for three individuals to share Ø Provides individualized supervision, personal care, and skill development training to individuals with high needs in an atmosphere that enhances self-image and worth. Ø Provides/encourages individuals to participate in household routines. Ø Provides community inclusion, budgeting, home management, etc. Ø Areas of support are assessed according to individual needs. |

Ø Individuals requiring intermittent services (less than twenty-four hours of service) to individuals who reside in their own home/apartment. Ø Provides skill development training to individuals so the individual may continue to live independently in a community setting. Ø Intervention is implemented as required. Ø Areas of support are assessed according to individual needs. |

Ø Provides opportunity and choice to individuals to participate in and contribute to their community. Ø Promotes exploring community resources, activities and events on an ongoing basis. Ø Provides the opportunity to practice individual Religious beliefs. Ø Provides the opportunity to explore employment and/or volunteer opportunities. Ø Provides the opportunity to explore educational opportunities. Ø Provides the opportunity to participate in recreational activities of their choice. |
